What is Endometriosis

Endometriosis is a condition that occurs when tissue that is similar to the lining of the inside of the uterus develops outside the uterus. At the end of the menstrual cycle, this tissue will disintegrate and bleed like ordinary uterine tissue does. However, the blood has nowhere to go, which causes areas nearby to swell or become irritated. This leads to lesions and scar tissue.

Types of Endometriosis

  • Endometrioma (ovarian lesion): Also known as chocolate cysts, they develop within the patient’s ovaries and are black and filled with fluid. These cysts can damage healthy tissues and they don’t respond very well to treatment.
  • Superficial peritoneal lesion: This is the most common type of endometriosis, whereby the lesions develop on the peritoneum (a thin layer of cells that lines the pelvic cavity).
  • Deep infiltrating endometriosis: This kind develops beneath the patient’s peritoneum and may affect the bowels or bladder, or other organs close to the uterus. It affects between 1% and 5% of women with endometriosis.

Causes of Endometriosis

  • Retrograde menstruation: This happens when menstrual blood containing endometrial cells flows back through the fallopian tubes and then to the pelvic cavity instead of out of the body.
  • Transformation of peritoneal cells: Hormones or immunological factors encourage peritoneal cells, which line the inside the patient’s belly, to change into endometrial-like cells.
  • Embryonic cell transformation: During puberty, hormones like estrogen may cause embryonic cells, which are still in the early stages of development, to grow into endometrial-like cell implants.
  • Immune system disorder

Risk Factors of Endometriosis

  • Having first menstruation at a young age
  • Heavy and prolonged menstrual periods
  • Higher levels of estrogen
  • Disorders of the reproductive tract
  • A history of endometriosis in the family

Symptoms of Endometriosis

Some of the common signs and symptoms of endometriosis include:

  • Chronic pelvic pain
  • Painful menstruation
  • Pain during intimacy
  • Fatigue that persists
  • Difficulty becoming pregnant
  • Unusual or substantial menstrual bleeding
  • Pain during bowel movement or when urinating
  • Blood in the patient’s stool or urine

Diagnosis of Endometriosis

To have a definitive diagnosis of endometriosis, the doctor may recommend the following tests:

  • Ultrasound & Other Imaging tests: Imaging tests such as an ultrasound, CT scan and MRI can give detailed images of the uterus and other organs.
  • Laparoscopy: The doctor makes a small incision in the patient’s belly and inserts a laparoscope, through which he/she can see the size and location of the lesions and perform a biopsy.

Endometriosis Treatment Options

There is no known cure for endometriosis, however different treatment options are employed in order to manage the symptoms. Treatments typically include surgery or medication.

Pain medication: An over-the-counter pain reliever may be recommended.

Hormone therapy: Hormonal therapy can halt a patient’s menstruation and reduce the amount of menstrual loss thus, improving the symptoms of endometriosis.

Laparoscopic & Robotic Surgery: The doctor may recommend surgery to remove the most afflicted tissue. Endometriosis surgery when performed by an expert, will alleviate the symptoms and result in longterm symptom relief with improvement in fertility.

Endometriosis is a condition where the tissue that lines the uterus (endometrium) grows elsewhere in the body. This is mainly in the ovaries, pelvis, and fallopian tubes.
